In order to access your Plan account information, you will be required to enter your system number, username, and password. Your access to your Plan may allow you to transmit personal information for your organization and its employees. By entering the site with your username and password, you consent to the electronic transmission of such information.
You are solely responsible for the confidentiality of your username, password and the information you access. You accept full responsibility for all transactions entered through your username and password. If your username and/or password are lost or stolen, or if you become aware that your username and password are being used without your authorization, you agree to notify us immediately.
