EPIC Forms

EPIC Application
The EPIC Application is used by retirement plan sponsors to establish access to the EPIC system. A pre-printed voided check or encoded deposit slip is required for "full access" employers.
EPIC Application - Third Party Administrator (TPA) Access
The EPIC Application – TPA Access is used by third party administrators to establish access to the EPIC system. A pre-printed voided check or encoded deposit slip is required for "full access" TPAs.
Traditional 403(b) Distribution Request
The Traditional 403(b) Distribution Request allows clients to request a distribution(s) from their Traditional 403(b) accounts provided they have a qualifying reason for distribution. This form can also be used to establish a systematic withdrawal from a Traditional 403(b) account.
Roth 403(b) Distribution Request
The Roth 403(b) Distribution Request allows clients to request a distribution(s) from their Roth 403(b) accounts provided they have a qualifying reason for distribution. This form can also be used to establish a systematic withdrawal from a Roth 403(b) account.
IRA/403(b) Designation of Beneficiary Form
The IRA/403(b) Designation of Beneficiary Form is used to designate, change or revoke a beneficiary on a client's retirement fund account(s).
Name Change / Correction
Use this form to correct any name discrepancy for an existing account, new account or when an account being transferred from another financial institution is held in a former/incorrect name. A name discrepancy may include a spelling error, use of an initial in place of a full name, use of a "nickname" or a new name due to a legal name change.
Required Minimum Distribution for 403(b) Accounts
The Required Minimum Distribution for 403(b) Accounts is used by clients who turn age 70 ½ and are required, by the IRS, to receive their RMD. The IRS requires 403(b) participants to distribute their RMD when they have attained age 70 ½, or retired from the employer sponsoring their plan, whichever is later.
Retirement Plan Investment Instructions
The Retirement Plan Investment Instructions form is used by participants when submitting new or updated investment instructions for 403(b), SIMPLE IRA, SEP-IRA, SARSEP-IRA account contributions.
Third Party Administrator (TPA) Authorization Form
The TPA Authorization Form is used to notify Foresters Financial Services, Inc. when a 403(b) or 457(b) plan sponsor employs the services of a third party administrator (TPA).
